LHC Group - - Responsibilities: Manage patient referrals and visit schedules; Utilize an automated scheduling system to maintain a calendar of services for episodic and per visit customers; Process workflow for requested scheduled, missed, rescheduled, reassigned, declined, and delivered visits; Monitor pending referrals daily and assign licensed professional and case manager for all start of care visits; Communicate daily with field staff regarding visits unaddressed in late, pending, or incomplete status for resolution as appropriate
